چکیده

Introduction Septic patients with myocardial depression are routinely treated with dobutamine [1]. Whether this strategy is desirable is questionable, as catecholamines increase cardiac work, reduce myocardial efficiency, and are cardiotoxic [2]. We can accurately predict mortality in a 72-hour fluid-resuscitated rat model of faecal peritonitis as early as 6 hours, based on the degree of myocardial depression (low stroke volume, high heart rate) [3]. This model offers a useful means of testing safety and efficacy of therapeutic interventions in predicted survivors and non-survivors.

Beta-blockade in experimental fluid-resuscitated sepsis: acute haemodynamic effects of esmolol differ in predicted survivors and non-survivors

Introduction Beta-blockade therapy during sepsis has a sound rationale in view of its cardiac, metabolic, inflammatory and other effects [1]. Whether it is safe and efficacious in both good prognosis and poor prognosis patients is yet to be ascertained. We have developed a 72-h fluid-resuscitated rat model of faecal peritonitis, where prognosis can be accurately predicted as early as 6 h post-i...

Influence of dobutamine on regional myocardial blood flow and ventricular performance during acute and chronic myocardial ischemia in dogs.

The data from this study document that dobutamine is a powerful inotropic agent in anesthetized dogs with acute myocardial ischemia and in awake, unsedated ones with chronic myocardial infarction. Dobutamine significantly increases heart rate at relatively small doses in anesthetized dogs with acute myocardial ischemia but considerably larger amounts of dobutamine are required to significantly ...

Myocardial impact and cardioprotective effects of apelin-13 and a c-terminal-modified analog during lps and clp experimental sepsis

Introduction Apelin-13 (APL-13) is a member of an endogenous peptide’s family (APLs) with powerful inotropic and cardioprotective properties. APLs bind to the dedicated receptor APJ-R, a member of the G protein-coupled receptor superfamily, all being widely expressed in the cardiovascular system. We have already shown that APL-13 infusion, was protective against LPS-induced myocardial dysfuncti...

Comparison of haemodynamic responses to dobutamine and salbutamol in cardiogenic shock after acute myocardial infarction.

Nine patients with critically reduced cardiac output after acute myocardial infarction underwent a single cross-over comparison of dobutamine and salbutamol to compare the haemodynamic effects of these drugs, which have, respectively, predominantly beta 1-adrenergic and beta 2-adrenergic agonist activity. The responses were used to select the more appropriate treatment for individual patients. ...

Haemodynamic effects of dobutamine with special reference to myocardial blood flow. A comparison with dopamine and isoprenaline.

The haemodynamic effects of dobutamine (2.5 to 10 micrograms/min per kg) were determined in 5 patients without cardiac failure who were undergoing cardiac catheterisation for suspected coronary disease. Myocardial blood flow was determined by the coronary sinus thermodilution technique. Data were compared with those from two groups of 5 patients who received dopamine (4-8 micrograms/min per kg)...
